Sign up form for SASLHA working group/Task team, SIG, Committee Chairperson or Committee Member

The SASLHA Bylaws indicate that volunteers on any SASLHA Working group/Task  Team/ SIG/Committee Chairpersons/Committee members will be appointed by the  SASLHA Council. The Council will consider volunteers to these roles according to the  needs and required skills of the specific groups/roles. The Council will also consider  any risks to the Association for reputational risk, financial risk, and any other relevant  risks considered on a case-by-case basis. The Council will also ensure that all  appointments of the volunteers to these roles are aligned with the SASLHA objectives  and values. 

All information will remain confidential and will only be used for record keeping and  contact purposes.

Please fill in all applicable sections, initial each page and sign at the  end of the application. 

This is a generic application form for all SASLHA volunteer roles. Please therefore complete the relevant sections for the role in which you are volunteering.

You will:

  • Maintain confidentiality about all discussions and information that you are privy to in this volunteer role unless permission has been given to share these.

  • Maintain an openness and willingness to engage with and learn from each other in a respectful and dignified manner.

  • Engage respectfully with relevant other volunteers and paid SASLHA members within the group and on social media.

  • State, when engaging on a public platform that you are expressing a personal opinion and not speaking on behalf of SASLHA unless expressly authorised to do so.

  • Keep yourself informed about the latest research and only quote what is current evidence.

  • Refrain from engaging on social media or any other communication platforms that may place SASLHA in any form of risk or disrepute.


You acknowledge that:

  • SASLHA may distance itself from anything you say publicly if it violates this agreement.

  • Failure to abide by these conditions may result in a termination of my appointment as a volunteer for a SASLHA activity.
